Full Time, Field-Based, Home Health Registered Nurse in San Diego,

Position Overview: The Registered Nurse (RN) provides skilled
nursing care for patients of all ages in their place of residence.
The RN coordinates care with the interdisciplinary team,
patients/families, and referring agencies, assuming responsibility
for overall coordination of care. Home Health Registered Nurse
Compensation The pay for this position is $52.00/hr Benefits are
available to full-time employees after 90 days of employment A
401(k) with a company match is available for full-time employees
with 1 year of service on our eligibility dates Home Health
Registered Nurse Highlights This position is a contract assignment
with potential to hire on permanently based upon attendance,
performance, and business needs The required availability for this
position is Monday-Friday 8am-5pm Home Health Registered Nurse
Responsibilities · Assess home health and/or hospice patients in
both stable and emergent situations to determine physical,
psychosocial, and environmental needs. · Develop, implement,
document, and evaluate individualized Plans of Care (POC) to ensure
quality and continuity. · Gather data on physical, psychological,
social, spiritual, and cultural factors impacting patient/family
health and integrate findings into the POC. · Maintain accurate,
up-to-date patient records, documenting all visits, contacts,
problems, plans, actions, and goals. · Provide care using infection
control measures to protect both staff and patients (OSHA
compliance). · Educate patients and families on care, diet,
medications, home safety, and self-care practices tailored to age,
condition, and culture. · Supervise and provide clinical direction
to HHA/STNA/LVN staff to ensure quality service. · Assess and
manage pain using appropriate interventions and educate patients
and families regarding pain management. · Coordinate referrals to
other healthcare agencies when necessary. · Monitor assigned cases
to ensure compliance with third-party payer requirements. · Provide
on-call services to patients and families as assigned. · Serve as a
liaison between patients, families, team members, administration,
physicians, and other healthcare professionals. · Participate in
interdisciplinary team meetings and other relevant meetings. ·
Perform other related duties as assigned. Home Health Registered
Nurse Requirements Required Skills & Qualifications Demonstrated
ability to assess and respond to patient and family needs across
varied settings. Strong communication skills at all levels of the
organization. Knowledge of medications, correct administration,
venipuncture, and IV care. Competency in wound care, inhalation
therapy, phlebotomy, and bowel/bladder training programs. Ability
to manage indwelling catheters and drains, use durable medical
equipment safely, and apply physician-ordered home restraints when
necessary. Strong organizational skills with the ability to set
priorities and manage stressful situations. Commitment to
professional growth and competency. Flexibility to work varied
schedules and travel locally. Ability to maintain confidentiality
and work both independently and as part of a team. Technical
Skills: Homecare Homebase software – required OASIS – required
Certifications & Licensure Current state license as a Registered
Nurse – required CHPN certification – preferred Current CPR
certification – required Valid driver’s license, reliable
transportation, and insurance – required Experience 1–2 years as a
Registered Nurse in a clinical care setting – required 2 years of
