      • Patient Care:
      • Work with geriatric patients
      • Complete Medicare annual health assessments at clinics and home visits
      • Locations: San Diego (North County)
      • Patient Load:
      • 7-8 patients/day in clinic
      • 3-5 patients/day on home visit days
      • This position requires working in the clinic and home visits
      • Clinical Responsibilities:
      • Perform H&Ps
      • Use preventative health screening tools (retinal cameras, A1c testing, kidney testing for diabetics)
      • Navigate and chart via EMR systems (eClinicalWorks and others)
      • This position focuses on completing comprehensive health assessments and collaborating with patients' primary care providers to ensure continuity of care. You will enhance patient outcomes through detailed evaluations and effective communication, but this is not a primary care or PCP role.Training:

          • Current NP license for the state of California
          • EMR systems and electronic documentation (preferred)
